Specialty Telehealth Care
Our Neuro Network provides specialty telehealth care for adults affected by acute, chronic, and complex neurologic and neurometabolic conditions. Clinical services are available to patients who are physically located in Hawaii or Arizona at the time of service.
Hawaii
Beginning April 2026, target launch, Our Neuro Network plans to participate with most major Hawaii commercial insurance plans, as well as Medicare and Medicaid, subject to final credentialing and payer approval. Prescribing of controlled substances via telehealth is subject to federal and Hawaii state law. Controlled substances may be prescribed only when legally permissible, clinically appropriate, and consistent with all applicable regulatory requirements.
Arizona
At this time, Arizona-based services may be out of network with commercial insurers. Prescriptions are limited to non-controlled substances at this time, but may be offered at a later date when clinically appropriate. Cash pay options may be available. Medicare and Medicaid services will be offered only where credentialing and regulatory requirements are met.
Clinical Provider
Clinical services are delivered by Tiffany Hoke, DNP, RN, RNP, APRN-RX, AGACNP-BC, SCRN, CNRN, a doctorally prepared, licensed, and board-certified Adult-Gerontology Acute Care Nurse Practitioner, Certified Neuroscience Registered Nurse, and Stroke Certified Registered Nurse with over twenty years of experience in acute neuroscience and neurovascular care.
Care is grounded in careful listening, longitudinal clinical reasoning, and thoughtful synthesis. Visits prioritize understanding the full clinical story, identifying meaningful patterns, and developing individualized care plans that respect both medical complexity and lived experience.
Care Model
Acute, chronic, and complex neurologic conditions are rarely understood or adequately addressed in a single visit. These conditions often require time, continuity, and ongoing clinical reassessment across a coordinated continuum of care.
Effective management frequently depends on close interdisciplinary collaboration with other specialty providers, which may include neurologists, neurosurgeons, primary care providers, physical therapists, occupational therapists, speech-language pathologists, and other members of the healthcare team as clinically indicated.
Care provided by Our Neuro Network is not a replacement for evaluation or treatment by primary care or other specialty providers. Rather, it serves as a thoughtful adjunct designed to support coordination, clinical synthesis, and individualized care planning across settings.
Whenever possible, Our Neuro Network facilitates appropriate referrals, communicates clinical impressions and recommendations, and collaborates with treating providers to optimize outcomes, support functional recovery, and improve quality of life for individuals living with acute, chronic, and complex neurologic conditions. This collaborative, interdisciplinary approach reflects the reality of neurologic disease and supports care that is patient-centered, evidence-informed, and responsive to evolving clinical needs.
Our Neuro Network operates within an insurance-based care model that supports thorough evaluation, appropriate follow-up, and ethical clinical decision-making. Visit recommendations are guided by clinical need rather than volume, speed, or administrative pressure.
Telehealth services are not appropriate for medical emergencies. If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency department. When clinically indicated, referrals to emergency departments or other acute care facilities may be made to ensure timely evaluation and treatment. Such referrals occur when a patient’s needs fall outside the scope of telehealth care and when in-person, urgent, or emergent evaluation is in the patient’s best interest.
Visits and Follow-Up Care
Initial and follow-up visits are scheduled based on clinical complexity and patient need. If a concern requires additional diagnostic reasoning, coordination, or prolonged discussion, follow-up visits may be recommended to ensure safe and comprehensive care.
Services are billed in accordance with applicable insurance coverage, payer policies, and standard medical coding practices. Patients are responsible for any copayments, deductibles, or coinsurance as determined by their insurance plan. For patients receiving care in Arizona where insurance coverage may not apply, available cash pay options will be discussed transparently prior to scheduling.
Secure Messaging via Spruce
Secure messaging via Spruce is available to established patients to support continuity of care between visits. Messaging may be used for clarification of recommendations, updates or newly available information, and limited care coordination.
Secure messaging is not a substitute for scheduled visits and is not intended for urgent or emergency concerns. Messages are typically reviewed within one to two business days during regular business hours.
Scope of Clinical Services
Clinical services may include, when appropriate, medical history review and clinical assessment, diagnostic evaluation and medical guidance, medication, laboratory, or imaging orders, completion of medical documentation or forms, and communication with members of the healthcare team.
All care is individualized and guided by clinical judgment, current evidence, and patient-specific needs rather than billing constraints.
Capacity and Availability
To ensure safe, attentive, and thoughtful care, appointment availability may be limited. New patient scheduling may pause periodically when clinical capacity is reached.
Licensure and Location
Telehealth care is provided only in states where appropriate licensure is held. At this time, telehealth care services are available to patients physically located in Hawaii or Arizona at the time of the visit, subject to licensure, payer participation, and regulatory requirements.
Coaching and consulting services are non-clinical, educational in nature, and available nationwide under separate agreements.

Insurance & Financial Policy
Insurance Participation and Payment
Our Neuro Network provides insurance-based clinical care with a model that prioritizes thoughtful evaluation, continuity, and clinical judgment. We participate with select insurance plans. Coverage and benefits vary by payer and by individual policy.
Patients are responsible for understanding their insurance benefits, including copayments, coinsurance, deductibles, and any non-covered services. Payment of patient-responsible amounts is due at the time services are rendered unless otherwise arranged. Insurance reimbursement is determined solely by the patient’s insurance carrier and the terms of the individual policy.
Insurance-Related Services and Limitations
Coverage determinations, authorizations, and reimbursement decisions are made exclusively by the insurance carrier. While we participate with insurance plans, Our Neuro Network does not:
Guarantee insurance coverage for visits, procedures, medications, laboratory testing, imaging, or other diagnostic services
Control payer determinations regarding medical necessity or reimbursement
Expedite care by reducing visit time, clinical documentation, or clinical complexity to meet payer-driven productivity metrics
Prior Authorizations and Administrative Processes
When clinically appropriate, Our Neuro Network may submit or support prior authorization requests and medical documentation required by insurers. Approval is not guaranteed and may result in delays that are outside of our control. Participation with insurance does not alter clinical recommendations, diagnostic reasoning, or individualized care planning.
Our Care Philosophy
Neurological and neurometabolic conditions are often complex, evolving, and deeply interconnected. Meaningful care requires time, careful synthesis, and continuity. Our Neuro Network is led by a solo practitioner working within an expert professional network. This structure supports continuity, accountability, and individualized decision-making while maintaining appropriate administrative boundaries.
While insurance participation improves access, our practice model remains patient-centered and outcome-focused, not volume-driven. Our Neuro Network intentionally practices in a manner that allows for:
Comprehensive, unrushed visits when clinically indicated
Thoughtful review of history, diagnostics, and prior care
Care plans guided by evidence, clinical judgment, and individual patient needs rather than visit volume targets
Out-of-Network Services and Documentation
For services that may be out of network or not covered by insurance, patients may be responsible for payment. When clinically appropriate, we may provide documentation or respond to insurer inquiries to support coverage determinations or patient-submitted claims. Submission of documentation does not imply or guarantee reimbursement.
Emergency and Urgent Care Disclaimer
Our Neuro Network does not provide emergency care. Patients experiencing urgent or emergent symptoms should seek immediate medical attention by calling 911 or presenting to the nearest emergency department.
